How do I make my first appointment?
Call your selected provider to make an appointment.
What if I want to get additional treatments?
Any additional treatments will get paid directly to the provider.

What day of the month will I be billed?
Every 30 days from the day of enrollment unless enrolled in the annual plan.
How do I update my payment information?
Payment methods can only be updated by signing in at club.evolus.com and add a new payment method.

CAN I CHANGE MY SELECTED PRACTICE?
Yes, you will have the ability to change your selected provider once a year.
How do I cancel my membership?
You can cancel at any time by signing in at club.evolus.com and clicking cancel membership. Cancellations will go into effect immediately.
If you cancel within the first three months a $100 cancellation fee will apply.
Your Club Evolus Membership is non-transferable, and payments are non-refundable.
How quickly do cancellations go into effect?
Immediately. You will not be charged on the next billing cycle. If you cancel within the first 3 months, there will be a $100 cancellation fee.
Can my membership be frozen or paused?
No, your Club Evolus membership cannot be frozen or paused.
Is there a cancellation fee?
If a membership is cancelled within three months of enrollment, a $100 cancellation fee will be applied.
If I cancel, can I reactivate my membership?
Yes, to re-activate your membership, sign in to club.evolus.com and reactivate on the membership screen.
Will I be able to get a refund if I cancel?
