General Questions

How does it work?
  • You will be charged $39 at enrollment, and then $39 recurring every 30 days.
  • You will be eligible for your first treatment after 3 payments of $39. Option to prepay for first treatment.
  • Your Club Evolus membership covers 20 units of Jeuveau per treatment, with a minimum of 90 days between each treatment.
  • Units do not accrue if you wait more than 90 days between treatments.
  • To maximize your savings, get treated every 90 days!
  • You will pay your provider directly for any additional treatments you receive.
  • Cannot be combined with any other offer.
  • Club Evolus members will be automatically opted out of Evolus Rewards upon enrollment.
When can I get my first treatment?
  • We have membership options that allow for immediate treatment or for treatment in 90 days.
How do I make my first appointment?
  • Call your selected provider to make an appointment.
What if I want to get additional treatments?
  • Any additional treatments will get paid directly to the provider.
Can this be combined wth Evolus Rewards™?
  • No, Club Evolus cannot be combined with any other offer.

BILLING

How will the payment show on my credit card statement?
  • Club Evolus.
What day of the month will I be billed?
  • Every 30 days from the day of enrollment.
How do I update my payment information?
  • Payment methods can only be updated by signing in at club.evolus.com and add a new payment method. Don’t forget to make this the default payment.
What payment methods are accepted?
  • Credit card, Apple Pay, and Google Pay

CANCELLATIONS

Can my memberships be transferred to another individual or another location?
  • No, memberships are non-transferable.
How do I cancel my membership?
  • You can cancel at any time by signing in at club.evolus.com and clicking cancel membership. Cancellations will go into effect immediately.
  • If you cancel within the first three months a $100 cancellation fee will apply.
  • Your Club Evolus Membership is non-transferable, and payments are non-refundable.
How quickly do cancellations go into effect?
  • Immediately. You will not be charged on the next billing cycle. If you cancel within the first 3 months, there will be a $100 cancellation fee.
Can my membership be frozen or paused?
  • No, your Club Evolus membership cannot be frozen or paused.
Is there a cancellation fee?
  • If a membership is cancelled within three months of enrollment, a $100 cancellation fee will be applied.
If I cancel, can I reactivate my membership?
  • If you want to re-enroll prior to the next billing date, you can reactivate your membership by logging into club.evolus.com.
  • If it has been past the next billing date, you would need to re-enroll.
Will I be able to get a refund if I cancel?
  • No, all payments are non-refundable.
