What is Club Evolus?

Club Evolus is the first-of-its kind patient membership program designed to maximize savings on frown line treatments. Visit club.evolus.com for details.

What membership types are offered?

  • Monthly: $0 at enrollment, then $49 every 30 days. The patient is eligible for their first check in 90 days after enrollment (i.e., after three payments have processed).
  • Prepay Monthly: $147 at enrollment, then $49 every 30 days. The patient is eligible for their first Club Evolus check in as early the day of enrollment. If the patient previously used Evolus Rewards, their first Club Evolus check in must be at least 90 days after their last Evolus Rewards check in.
When is a patient eligible for their first Club Evolus check in?

  • Prepay Monthly: The patient is eligible for first check in as early as the day of enrollment. If the patient previously used Evolus Rewards, their first Club Evolus check in must be at least 90 days after their last Evolus Rewards check in.
How often can a patient eligible for a Club Evolus check in?

What if a patient wants additional services?

Patients will pay you, the provider, directly for any additional treatments or services.

Can a patient use their membership at another practice?

No, a Club Evolus membership only works at one practice at a time. Club Evolus is designed to be a membership at your practice.

Can Club Evolus be combined with Evolus Rewards?

No, patients can only be enrolled in one program at a time. Offers cannot be combined. Patients will be automatically opted out of Evolus Rewards upon Club Evolus enrollment. Patients will be re-enrolled in Evolus Rewards upon Club Evolus membership cancellation.

What if a patient needs help?

Patients can use the chat feature at club.evolus.com to chat with a live Evolus representative.

USING CLUB EVOLUS

How do I access the Club Evolus program?

Club Evolus can be accessed through both the Evolus Practice App (downloadable from the AppStore or Google Play Store) and My Evolus portal at myevolus.evolus.com. All My Evolus logins have access to enroll and check in patients. My Evolus admin logins also have access to pause enrollments, view reports on the Rewards Dashboard and redeem Reward Vials.

Can I pause enrollments?

Can pause new enrollments within the MyEvolus Rewards Dashboard by toggling “New ClubEnrollments” to the off state on the “Patients Enrolled” tile. Active memberships will still continue and be able to check in and no new enrollments will be accepted while paused. You can un-pause anytime by toggling “New Club Enrollments” to the on state.

Can I track enrollments and cancellations?

You can track the number of patients who enroll and cancel club evolus in the MyEvolus Rewards Dashboard on the “Patients Enrolled” tile or by downloading an XLSX file on the“Club Evolus Eligibility” tile.

Can I see who is eligible for their next check in?

You can see who is eligible for their next check in by downloading an XLSX file on the “Club Evolus Eligibility” tile on the MyEvolus Rewards Dashboard.

Can I see who has failed payments?

You can see a list of patients with failed payments in the MyEvolus Rewards Dashboard by downloading an XLSX file on the “Club Evolus Eligibility” tile. You will also see that the patient is not eligible for check-in due to failed payment if you check the patient’s status through the check-in process in MyEvolus.

PAYMENTS

When are payments processed?

Payments are processed every 30 days, beginning from enrollment. If a patient chooses the Prepay Monthly option, they will be charged $147 at enrollment.

How does a patient update their payment method?

Patients can manage their membership and payment method anytime by signing into their patient portal at club.evolus.com.

Can I update a patient’s payment method?
Up arrow

No, payment methods can only be updated through the patient portal at club.evolus.com.

What payment methods are accepted?
Up arrow

Visa, Mastercard, American Express, and Discover credit cards.

How will the payment show on a credit card statement?

Credit card statements will show a charge from “Club Evolus”.

What happens if a payment fails?

If a payment is not processed within 15 days of the original attempt, the membership will result in a forced cancellation.

When a payment fails, the patient will be sent an automated series of email communications to update their payment method. The patient will not be eligible to check-in until their payment method is updated. Patients can update their payment method through their club.evolus.com patient portal at any time.

CANCELLATIONS

How does a patient cancel their membership?

Patients can cancel at any time by signing into their patient portal at club.evolus.com and navigating to the Membership page.

How quickly will a cancellation go into effect?
Up arrow

Cancellations will go into effect immediately. The patient will not be charged on their next billing cycle and will not be able to check in for another Club Evolus treatment.

Is there a cancellation fee?

Yes, if a membership is cancelled within three months of enrollment, a $100 cancellation fee will apply.

Can a membership be transferred to someone else?

No, Club Evolus memberships are non-transferable.

Can a member get a refund?

No, Club Evolus membership payments are non-refundable.

Can a membership be frozen or paused?

No, Club Evolus memberships cannot be frozen or paused.

How does a patient re-enroll after cancelling?

Patients can re-enroll by signing into their patient portal at club.evolus.com and navigating to the membership page or by a practice initiating through MyEvolus.
