Club Evolus can be accessed through both the Evolus Practice App (downloadable from the AppStore or Google Play Store) and My Evolus portal at myevolus.evolus.com. All My Evolus logins have access to enroll and check in patients. My Evolus admin logins also have access to pause enrollments, view reports on the Rewards Dashboard and redeem Reward Vials.
Can pause new enrollments within the MyEvolus Rewards Dashboard by toggling “New ClubEnrollments” to the off state on the “Patients Enrolled” tile. Active memberships will still continue and be able to check in and no new enrollments will be accepted while paused. You can un-pause anytime by toggling “New Club Enrollments” to the on state.
You can track the number of patients who enroll and cancel club evolus in the MyEvolus Rewards Dashboard on the “Patients Enrolled” tile or by downloading an XLSX file on the“Club Evolus Eligibility” tile.

You will also see that the patient is not eligible for check-in due to failed payment if you check the patient’s status through the check-in process in MyEvolus.
Payments are processed every 30 days, beginning from enrollment. If a patient chooses the Prepay Monthly option, they will be charged $147 at enrollment.
Patients can manage their membership and payment method anytime by signing into their patient portal at club.evolus.com.

If a payment is not processed within 15 days of the original attempt, the membership will result in a forced cancellation.
When a payment fails, the patient will be sent an automated series of email communications to update their payment method. The patient will not be eligible to check-in until their payment method is updated. Patients can update their payment method through their club.evolus.com patient portal at any time.
Patients can cancel at any time by signing into their patient portal at club.evolus.com and navigating to the Membership page.
Cancellations will go into effect immediately. The patient will not be charged on their next billing cycle and will not be able to check in for another Club Evolus treatment.
Yes, if a membership is cancelled within three months of enrollment, a $100 cancellation fee will apply.
